Overview How Duodenal Switch Works
The first step of a duodenal switch is performing a sleeve gastrectomy. This reduces the size of the stomach by up to 80 percent, reducing the amount of food a patient can consume during a meal.
Next, approximately half of the small intestine is altered and bypassed. This reduces the number of calories that are absorbed during the digestion process, leading to malabsorptive weight loss.
The bypassed portion of the small intestine is rerouted and connected to the other part of the small intestine. This ensures bile and digestive juices from the pancreas can still aid in digestion.
The SIPS Technique SADI-S vs. BPD-DS
SIPS stands for stomach intestinal pylorus sparing. This is a variation on the traditional duodenal switch (BPD-DS) known as single anastomosis duodeno-ileal bypass with sleeve gastrectomy (SADI-S) or single loop DS. SADI-S maintains more of the small intestine, and transects it at one point rather than two. Dr. St. Laurent performs SIPS/SADI-S because it reduces the risk of complications.

Dr. St. Laurent presents a detailed overview of a laparoscopic loop duodenal switch procedure. Through an incision on the left side of the patient, Dr. St. Laurent dissects the lower portion of the stomach before dividing the duodenum to achieve duodeno-ileal anastomosis. After inserting a Jackson Pratt drain on the lower border of the stomach to remove any leftover abdominal irrigation, the abdomen is closed with sutures.
